Note: It is your responsibility to pay for services and there is no guarantee that your insurance will pay for treatment. If you use insurance, then your payment is based on your coverage and benefits. You can verify your current benefits by calling the number on the back of your insurance card.
It is your responsibility to communicate with me and any third-party processing your claims if your insurance or plan coverage changes. Not providing sufficient insurance information may result in being billed out-of-pocket.

My current self-pay fees are listed above; if I increase my fees, I will notify any active clients 60-days before the change. For self-pay clients referred through a third-party platform, like Sondermind or Headway, see the their website for current self-pay rates.
Cancelation Policy
No Surprises Act
If you are unable to attend your appointment, please call at least 24 hours before your scheduled appointment.

Cancellations after the 24-hour time period will be considered a no-show and are subject to a cancellation fee of $80.

You are entitled to receive a “Good Faith Estimate” that provides the cost of your medical treatment expenses.
Under the law, health care providers are required to give patients who uninsured or who don't use insurance an estimated bill for medical treatment expenses.

You have the right to receive a Good Faith Estimate for the total estimated cost of any non-emergency expenses, including related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical treatment. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule any treatment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.
